The Transport Accident Investigation Commission (TAIC) has released a report outlining the causes of the accident and providing recommendations for improvement. The investigation found that the master navigated mainly by eye, relying on visual cues rather than using electronic instruments, which limited their ability to confirm location and heading.
This approach proved risky in challenging conditions, where reduced visibility restricted visibility. The deckhand's warning of impending danger was insufficiently acted upon due to time constraints imposed by excessive speed and route choice.
The accident resulted in minor injuries to two people on board, but fortunately no pollution occurred. Coastguard later towed the vessel off the rocks and back to port.
The TAIC report serves as a reminder for marine professionals to use all available navigation aids, cross-check between visual cues and electronic instruments, set safe speeds, standardize routes and passage plans, and implement regular proficiency checks and refresher training for launch and pilot-vessel masters. South Port has taken steps to address these issues by updating its training and procedures, adding six-monthly proficiency assessments, and revising its Maritime Transport Operator Plan and standard operating procedures.
